Not too sue how to explain but why does the padding bunch up unevenly around the rear as its really u comfortable when uneven when you sit feels like it's the material inside the molicare mobile (pull up). I have had simular issues in the past with Tena Pants and Depend pull ups also in the hotter weather I get a lot of chaffing, and I also like I snugg fit.

My issue apparently comes from the nerve issues in the L5 impingement and not caused by the bladder I have been told but experience leakage and the bladder having a mind of its own.

I have anxiety issues in general so mentioning this to my support network G.P, Mental Health Nurse, Psychologist (even tho bladder issues are in my file says Destrusor Instability and Urgency) is just impossible for me and not under urologist as tests show its not from the bladder but back also seen Neurosurgeon and nothing he can do for me either and surgery on my back is completely ruled out by specialists years ago too risky for so little or no positive return.

I have also rang up places that have continence nurse but calls never get returned and my anxiety disorders make it nearly impossible to follow up as was nearly impossible to ring in the 1st place. :(

I'm not sure where to start in an effort to address your concerns. I don't understand many of the things you talk about regarding the medical aspects of your incontinence (IC), but from what I "think" I understand from what you talked about it sounds like maybe your IC might have something to do with some type of mental stress issues and is more mental (PTSD ???) compared to physical, or nerve related issues of the neck, or spine. I don't understand what you mean when you say, "L5 impingement". If the L5 nerve, or nerve root has been damaged you will have MORE symptoms than just urinary IC. L5 nerve damage can usually be identify pretty easily with the use of a nerve study test, MRI, and other tests preformed by a neurologist, and neurologist. The urologist can measure lack of bladder function, while the neurologist measures the extent of damaged nerves usually by measuring nerve pulse, and conformation with an MRI, and other tests.

What country are you located? If your in the land of funny flushing toilets (Europe), particularly if your located in the UK, the way medicine is administered is very different compared to the USA. But if you are located in the USA, and have an honorable discharge from the military then you could receive FREE medical services from the Veterans Affairs commonly referred to as the "VA" as a means of identifying the cause(s) of your IC. Much more information is needed for me to even comment on what "I" believe might be some possible cause(s) of your IC.

The bunching up, or clumping of the pulp as I call it found inside the diaper is a common problem for anyone that wear a disposable diaper for an extended period of time. As you probably already know, the diaper does not have to be wet for this material to start clumping together in the back of the diaper. There are only two things I've ever found to improve the situation and that is try buying a higher quality diaper, and/or change the diaper more frequently. The ladder is the best especially if the diaper is wet. Excessive diaper movement can cause the diaper to break down and form clumps in the back of the diaper especially if wet. I think the most common cause for diaper break down is prolonged wear by the user, or inexpensive brand diapers will also fall apart faster than quality made disposable diapers.

The IC is caused by the spinal cord issues I have been told as had worse anxiety issues before the incident with the back in 2009 I also have osteoarthritis and lumbar pain which I see a Physiotherapist for on a fortnightly basis and since seeing the Physio regularly the IC issues decreased a little.

The reason I mentioned the anxiety disorders was to try explain why I depend on help from forums etc

I am from Australia and Molicare tends to be the main "premium" brand here :(

Molicare is a quality made diaper. I don't know if it is "the best" disposable diaper on the market, but the "best" diaper you can use will be the one that doesn't leak, and shows no bulk, and contains odor. What else can any user what?

I used Molicare Super Plus for a long time and found them be good quality diapers. I honestly don't think any of the other leading diaper manufactures would be any better, or any worse when it comes to diaper break down. I have to think the best advise I can give is that you change the diaper more frequently to avoid diaper break down especially if the diaper is wet, and you have a lot of body movement while wearing the diaper. Just my thoughts on the matter. Others may have a different opinion.

Molicare diapers are a good choice, there not my choice but I use them and they did a good job. Im just looking at what you wrote it sounds like you are not changing soon enough. Do to my condition clump and break down any diaper if I push it to far. I know diapers are very expensive but it sounds like you need to change sooner and air out more. Damage to the L-5 area can and will make your bladder do crazy stuff. Hope this helpful

Clumping of the padding in disposable diapers is something that varies hugely amongst brands. Abena and ComfiDry 24/7 are two brands that minimize clumping. Wearing a pair of snug underpants over the diaper also helps.

Ah, the old L5 problems. That seems to be a more common point which can lead to urinary problems. Mine was spinal shock and bruising to the L5-S1 area. I can still walk and have full feelings in my feet, but have also had other problems related to that area of my spine. I too have ruled out spinal surgery which probably won't help me either (and could potentially make things worse). I've recently discovered a condition called detrusor sphincter dyssenergya (dsd), which seems to fit my symptoms completely. You may want to do some searching on this, as what you describe sounds loosely familiar.

For the clumping I also second on wearing better diapers, or if that isn't possible then snug underwear or compression shorts. (Molicare supers will be better than molicare pullups too). Part of the problem is the manufacturer using thinner/looser packed padding, and part is being physically active and moving/bending a lot. Sweating in a diaper seems to make it worse than peeing too, though I'm not fully sure why. Other than that, yeah you will just need to change more often.
