I would just mention that I have a small disability.
The last thing you need is someone that love the diapers
and not you just for being you.
If they accept the thought of a minor disfunction,
they are prepared.

It is not something to even mention on a dating profIle really.
Injuries and limitations and disabilities really shouldn't be discussed until like the 4tb or 5th date in general.

I am not in the dating scene but I do think that you would not put on your profile "I wear boxer shorts, or jockey shorts". By the same token you do not need to tell that you have to wear a diaper.
