Does anyone know of any brands that have their larges starting at size 40 or 42 inch waists that im not aware of?

I don't understand this. Are you saying your insurance won't cover the cost of larger diapers because you're too small, or because you feel you're too small for the diapers. Diaper sizing is just a generally recommended range, which varies from person and from manufacturer. If you're a 42 and the size on one brand starts at 40 (and goes higher), then you should be just fine. Likewise, if another brand starts at 44, you're probably just fine for them as well.
